Project Food Box flyer and Tangelo

Hello. We have been informed about the free meals offered by numerous organizations, which are part of this larger project called CalAIM. Please note that this is a one-time only offer and really intended for those who are recuperating from a medical procedure or need help in recovering from an illness.  If someone takes advantage of the free meals while they are healthy, they might regret not having this available to them when they are really ill and need the help.

Individuals with Medi-Cal can apply if NOT already enrolled in any other food program such as The Project Food Box.  Enrollment can be completed on-line and it is very easy.  The app can be downloaded. Once eligible, they get money in their Tangelo wallet and the individual/family can shop in the Tangelo Marketplace, and groceries are delivered to their home.  Individuals who do not have medical insurance can apply if they were hospitalized or discharged from a nursing facility or at risk for entering. Please see the website for further details.  More than one individual in the home can apply if they meet the criteria. 
 
The Project Food Box
This program is funded by CalOptima for anyone receiving Medi-Cal services.  The initial authorization is for 12 weeks and then a new authorization needs to be submitted for another 6 weeks.  The family will receive a written notice prior to the re-authorization to contact The Project Food Box.  A nutritionist is also involved in the selection of foods based on the individual’s needs.  In order to qualify, the individual must have a medical diagnosis (Autism, Down Syndrome…) and a chronic medical condition such as diabetes, overweight, cholesterol, or borderline.  Please feel free to reach out to The Project Food Box for further questions.
